These are the classic symptoms of an intake vacuum leak. Other causes could be:
* Needs a tune up. (clogged air filter, but also bad plugs and wires and/or clogged fuel filter)
* EGR valve stuck part open.
* Bad fuel pressure regulator. (EFI only)
* Blown or leaking head gaskets.
* Loss of compression from stuck or scarred piston rings.
* Loss of compression from a leaking or cross-threaded spark plug.
I suggest doing a full tune up (air filter, fuel filter, plugs, wires, PCV valve, cap and rotor) and seeing if that helps. After that, check for vacuum leaks. You'd be doing yourself a favor if you'd check the EGR valve at the same time.
